The government exchange or marketplace that you received your insurance from should issue you a
Form 1095-A. If you are the policy holder and haven't received your
Form 1095-A, you should contact the state or federal marketplace you enrolled with and request your
Form 1095-A.
Under some circumstances the Marketplace will provide
Form 1095-A to one taxpayer, but another taxpayer will also need the information. The person who received the
Form 1095-A should provide a copy to the other taxpayer that needs the information.
Note: If you were enrolled in Medicaid after applying for coverage through the Marketplace, you won't receive
Form 1095-A for your Medicaid coverage.
