What do I need to submit to New Mexico in order to claim the New Mexico Child Day Care Credit?

A separate Form PIT-CG should be completed by each caregiver who provided day care services for which a credit amount is being claimed.

If you made all reasonable attempts to complete Form PIT-CG, and you are still unable to locate the caregiver or to obtain the required information, complete Parts I and II of Form PIT-CG based on previous billings or other records, provide the name and address of the caregiver, and explain why the caregiver did not complete the statement.

When filing electronically, you should submit the completed New Mexico Child Day Care Credit Worksheet and Form PIT-CG along with New Mexico Form PIT-8453 to the address on PIT-8453 as soon as possible. We'll show you how to print and mail Form PIT-8453.

When not filing electronically, you must attach the completed New Mexico Child Day Care Credit Worksheet and Form PIT-CG to Form PIT-1 and Form PIT-RC in order to claim the New Mexico Child Day Care Credit.
