Form 3922

If you purchased ESPP stock (Employer Stock Purchase Plan) your employer will send you Form 3922. The Form 3922 isn't entered on your tax return. The information on Form 3922 is used to help you know the cost basis and the date of purchase of the stock for when you sell the stock and are reporting the stock sale on your tax return.

When you do sell the stock, you will receive a Form 1099-B or consolidated 1099 that shows the sales price and often will show the cost basis. You can then compare the cost basis on the Form 3922 with the cost basis on the 1099-B or consolidated 1099 to make sure the cost basis on the 1099-B or consolidated 1099 is correct.
