What if Box 16 or Box 19 of my 1099-R is blank?
If it's blank, that's ok. That just means whoever issued your form didn't know the state / local distribution amount or you didn't have any state / local distribution amount.
For most states, you don't need to enter anything here, or you can enter the same amount from Box 1, Gross Distributions here.
If Box 16 has the state name or abbreviation enter the state in Box 15, State on the screen.
