Where do I report rollover contributions shown on a Form 5498?

You do not report Form 5498 on your tax return because you are already reporting the rollover distribution from your 1099-R.

Form 5498 reports if you rolled over a pension account or some other IRA account into a different IRA account. You should have received a 1099-R for the old pension plan from the former trustee of your account. The 1099-R is the only form you report to show the rollover. You would not report the amount from the Form 5498 because it is the same rollover; it is just reporting it from the new account's perspective.
