What documents do I include with my Georgia return if my name changed since last year?

The Georgia Department of Revenue requires a copy of one of the following documents plus a copy of a photo ID reflecting name change.

Legal document reflecting name change:

• Superior Court Order/Decree/Certificate
• Marriage Certificate/Divorce Decree
• Social Security Card

Valid photo ID issued reflecting name change:

• US State or US Territory issued ID card (front and back)
• US Passport
