What qualifies as 100% disabled?

You're considered 100% disabled if you're unable to engage in any substantially gainful activity because of any medically determinable physical or mental impairment that can be expected to result in death (or has lasted or can be expected to last for a continuous period of no less than 12 months).

You don't have to be gainfully employed prior to such a disability to qualify.
