Do NOT enter any amount of your 2014 Indiana Income Tax withheld by your employer(s) as shown on any wage or tax statements.

Estimated Payments: If you did not have enough Indiana income taxes withheld from your earnings during 2014, you may have made estimated tax payments. These payments count as a credit toward your 2014 Indiana tax liability.

Extension Payment: If you filed for an extension to submit your income tax return, you may have made a tax payment with that extension. This payment counts as a credit toward your 2014 Indiana tax liability.

  • Enter any estimated tax payments you made for 2014.
  • Enter any amounts credited from your 2013 return.
  • Enter any amount prepaid with extension requests.
